Also, I've never heard of this rule ever being applied from anyone else.

Actually, in the 5-6 years that I played chess, I can only remember a handful of times that the initial position was set up incorrectly, and none of them occurred in tournaments. I think in those cases it was just fixed to what it was supposed to be. I guess if it is really indeterminable where the pieces were supposed to be given the correct initial position then the new position would just stand.
